Under the provisions of Section 4, Act 329 P.A. 1969, permission is hereby granted this application to burn the described address providing there is 2 acres of property and period specified below. PERMITS ARE NOT VALID FOR MORE THAN FOUR (4) CONSECUTIVE DAYS.

Please note that no fire shall be set after expiration of this permit, and no fire shall be set at a time when high wind is blowing. An adequate fire line shall be made around the area to be burned and a force of persons, sufficient to control the fire, shall be on hand at the time of burning.
